Subscription Form We have tried to make it easier for people with lung problems to join by making two categories of membership; one for individuals and one for health care professionals, hospital departments and health related companies. The annual donations requested are shown below. If you send in a tip to our Sharing the Health column, a picture of yourself living with lung disease, write an article or other contribution to help others, all yearly membership fees are waived. Membership Level with Annual Donation and Benefits Individual (For the person with Chronic Lung Disease) - With any donation from $10 to $35 you will receive our newsletter every other month for one year. Individual Contributor Level - A donation of $36 and more Health Professional - A donation of $40 receives one year (6 issues) of our newsletter. Health Professional Sponsor - A donation of $200 receives 25 copies of each newsletter. Health Professional Patron - A donation of $350 receives 50 copies of each newsletter. Subscription Form
